Patient privacy is a priority at Norwood Hospital. Your medical record information is confidential and access is limited to the patient and authorized individuals. An authorization to release your medical record, also called protected health information (PHI), must be signed by the patient, or the patient's legal representative (guardian or person with durable power of attorney). If a child is an emancipated minor or age eighteen years or older, parents cannot get copies of their child’s medical record.
To receive a copy of your medical records, please complete an Authorization to Use and Disclose Protected Health Information form.
